PT-141 (Bremelanotide)
Cyclic Melanocortin Receptor Agonist · Institutional Reference
Overview
PT-141 (Bremelanotide) is a synthetic cyclic heptapeptide analog of alpha-melanocyte-stimulating hormone (α-MSH). It is the first FDA-approved medication that acts on the central nervous system's melanocortin receptor system to address hypoactive sexual desire disorder (HSDD) in premenopausal women.
Mechanism of Action
Bremelanotide activates melanocortin-4 receptors (MC4R) in the hypothalamus, triggering intracellular cAMP signaling. This cascade modulates dopaminergic and oxytocinergic pathways within the limbic system, engaging centrally-mediated responses rather than peripheral vascular mechanisms.
Receptor Pharmacology
· Primary target: MC4R (melanocortin-4 receptor)
· Secondary target: MC3R (melanocortin-3 receptor)
· Receptor type: G-protein coupled receptor (GPCR)
· Signal: Gαs → adenylyl cyclase → cAMP → PKA
Pharmacokinetics
Key Neural Pathways
· Paraventricular nucleus (PVN) of hypothalamus
· Medial preoptic area (MPOA)
· Ventral tegmental area (dopaminergic)
· Nucleus accumbens (reward circuitry)
· Spinal autonomic pathways
Noted Observations (Clinical)
Phase III trials (RECONNECT) demonstrated statistically significant improvements in desire and reduction in distress scores versus placebo. Most common adverse events included nausea, flushing, and headache.
